The Palliative Approach in Long Term Care Community of Practice is a group of professionals, patients, caregivers and other interested individuals who come together to share experiences and best practices regarding Advance Care Planning and palliative approaches in LTC education and implementation- including challenges and successes. The group is run in collaboration with SPA-LTC and the Canadian Hospice Palliative Care Association.
The September 14, 2023 event will focus on 2SLGBTQ+ resources and support in Long-Term Care with guest speaker Clare Freeman. A non-profit executive leader and social work clinician with twenty years in residential care settings for a shelter and then hospice, Clare brings extensive expertise in organizational change management within the unionized workplace, equity & inclusion, strategic leadership, research, policy, fund development, and staff engagement. The session runs from 11AM to 12PM.
