Our Cancer Toolkit was developed through a partnership between the Advance Care Planning in Canada initiative, the BC Cancer Agency and members of the BC Patient Voices Network.

Cancer and Advance Care Planning: Tips for Oncology Professionals – a tool for oncology professionals that provides practical suggestions for when to have the discussion, with specific conversation starters that can help professionals get started.
