The purpose of this conversation guide is to assist Health Care Providers (HPC) in facilitating conversations with families and offer guidance on how to respond to questions regarding palliative care. The conversation guide is designed to accompany any of the corresponding Illness Trajectory Pamphlets but may be used independently as deemed appropriate by the HCP.
