The purpose of these documents is to increase the confidence and knowledge of staff caring for residents who are living and dying in Long-Term Care (LTC). It is intended for PSWs who assist in end-of-life decision-making within long-term care. Encouraging an interdisciplinary team approach in learning strengthens teamwork, awareness, and overall quality of care for residents in LTC.
This resource was developed by Advance Care Planning (ACP) in Canada, in partnership with the Strengthening a Palliative Approach in Long-Term Care (SPA-LTC) national team at McMaster University as part of a suite of resources to support long-term care professionals, residents and their families.
