All On The Same Page
The All on the Same Page video aims to provide an overview of a successful and meaningful case conference and highlights how Advance Care Planning informs these discussions. It is meant for residents, their families, and long-term care staff, with the ultimate goal of ensuring everyone is on the same page about what matters most to the resident.
