The Resource Guide for Home and Community Care supports healthcare professionals involved in home-based care environments across Canada. It provides a map of available resources to engage home and community care providers in three target behaviours for successful Advance Care Planning:

  1. Facilitating early and ongoing advance care planning conversations
  2. Understanding patients’ care wishes
  3. Reflecting wishes in the care plan and legal considerations

The Guide was developed by the Canadian Home Care Association for the Canadian Hospice Palliative Care Association (CHPCA) on behalf of the ACP in Canada project, in consultation with subject matter experts from across Canada.

It is designed for a variety of stakeholders involved in home and community-based palliative care, including home care providers, policy planners, program developers, educators and team leaders.
