The Ministry of Health and Long Term Care, Toronto Central Local Health Integration Network (TC LHIN) and East Toronto Health Link (ETHeL), are pleased to announce the formal launch of their Advance Care Planning e-Learning Modules: My Health. My Wishes. My Plan: An Advance Care Planning primer and practical approaches for healthcare providers. Input was gathered from patients, care-givers, physicians, nurses, community support staff, social workers and administrators. It is important to note that these e-Learning Modules underwent several levels of scrutiny, including checks for consistency with Ontario’s legal framework and privacy legislation. It has been reviewed by Speak-Up Canada as well as several individuals who spent many hours on providing significant content feedback for the current tool. Although the modules are geared toward expanding ACP knowledge and communication skills of health care providers (it will be CME accredited), it is relevant and useful for any member of the public, including patients with complex needs, care-givers, community agencies, and general public.
