Advance Care Planning conversations are not limited to Canada. These conversations can happen at any time, with anyone, anywhere in the world. Living in a digital age, it is even easier to have the conversation with people around the world, but very few do. Whether you have a grandparent living in Asia, a sister living in the United States, or a cousin living in Australia, you can talk to them about their future health and personal wishes.

Every fall, Advance Care Planning in Canada encourages people around the world to engage in Advance Care Planning conversations. The goal of this campaign is to normalize end-of-life conversations and showcase the organizations that are providing ACP resources around the globe.

This #ACPanywhere photo was taken by Chad Hammond, Project Manager ACP in Canada. Reflecting on his values and wishes, “I have come to Beechwood Cemetery, not because of the subject of my reflections but because it is a place of peace, beauty, and compassion. It is countless histories etched in wood and stone. In a world of digital memory, I find Beechwood a more tangible focal point to think about what matters most to me”.

This #ACPAnywhere photo comes to us from FACCA INC. in Essex County. The team featured is constructing the last bridge on the Rt. Hon. Herb Gray Parkway, and the entrance point to the new Gordie Howe International Bridge linking two countries – Canada and the United States. What does a bridge have to do with Advance Care Planning?, FACCA INC: when it comes to building bridges, “a little progress each day adds up to big results”. The same can be said for Advance Care Planning. You don’t need to decide what’s right for you in one fell swoop – you can do it in steps.
