The Palliative Approach in Long Term Care Community of Practice is a group of professionals, patients, caregivers and other interested individuals who come together to share experiences and best practices regarding Advance Care Planning and palliative approaches in LTC education and implementation- including challenges and successes. The group is run in collaboration with SPA-LTC and the Canadian Hospice Palliative Care Association.

The June 8, 2023 event will focus on Enabling Quality Intellectual and Developmental Disability Palliative Care: “Good Practices” with guest speakers Tracey Human, RN CHPCN(C) PPSMC, Director, Palliative Pain & Symptom Management Consultation, Toronto Program, Angela Gonzales, RN MN, Health Care Facilitator, Nurse Professional Practice Lead, Community Networks of Specialized Care – Toronto Region, Surrey Place, Janet Elder RN , BScN, CHPCN (c), PPSMC, SW Program, Windsor/Essex County, and Bob Parke, Consulting Bioethicist. The session runs from 11AM to 12PM.
