My Advance Care Plan – Regina Qu’Appelle Health Region
This form includes the person’s name, the name/contact information of their proxy and the names of others who have copies of their advance care plan. It indicates that they have discussed their advance care plan with their proxy and have given him/her the authority to speak for them if they become unable to communicate for themselves or are unable to understand what the care providers are saying. It also includes information regarding the person’s health care wishes – that is what they want – considering life support with medical interventions.

My Advance Care Plan – Proxy Form – Regina Qu’Appelle Health Region
This form is designed to be to the person’s doctor, Proxy, nearest relative, and/or trusted friends. It includes the person’s name, the name/contact information of their proxy and the names of others who have copies of their advance care plan. It indicates that they have discussed their advance care plan with their proxy and have given him/her the authority to speak for them if they become unable to communicate for themselves or are unable to understand what the care providers are saying.

My Life Capsule – Regina Qu’Appelle Health Region
The My Life Capsule program makes needed medical information available to EMS when they arrive at a home. The capsule is plastic and contains the patient’s medical information. It is kept in the patient’s refrigerator.
Patients put a “MY LIFE CAPSULE” magnet on the front of their fridge to alert EMS that the capsule containing their medical information is inside.

This workbook contains useful information on how to make the decisions that are right for you: how to hold the discussions with your loved ones that are a necessary part of the preparation of your plan for the future, and how to inform your doctor and nearest relatives of your wishes.
