Since 2012, the National Advance Care Planning Community of Practice has been supported by Advance Care Planning in Canada.
With a membership of just over 100 people, we meet virtually six times yearly to share resources, hear from expert speakers, and engage in dynamic dialogues about current ACP topics.
From Nova Scotia to Quebec, British Columbia to Ontario, PEI, and the Yukon, our members include:
- Regional health authority clinicians in Alberta Health Services, Saskatchewan Health Authority, Nova Scotia Health Authority, etc.
- Community organizations such as Nova Scotia Hospice Palliative Care Association, and Punjabi Community Health Services.
- Victorian Order of Nurses Canada.
- Institute for Healthcare Improvement.
