This year was a huge success thanks to the many people and organizations who partnered with us to promote advance care planning!

An infographic was also created to showcase National Advance Care Planning Day’s impact.

Our ACP Day 2017 campaign’s theme is “My Community.” The campaign kit tools will remind individuals to think about the people in their community and suggest ways to have conversations and decide on a Substitute Decision Maker.
