This year’s theme for National Advance Care Planning (ACP) Day is “My ACP Team.” Talking with your Substitute Decision Maker is essential, but it’s also important to talk with others who will support you and your Substitute Decision Maker when you need healthcare, to help prevent potential conflict during a difficult time. Our annual “Speak Up” campaign ask Canadians this year: who is on your team, and what vision for your care do you want to share with them?
